Every programme begins with a detailed assessment. Our physiotherapists look at how you move, where the pain or weakness comes from, and what you need to get back to — whether that is climbing stairs comfortably, returning to sport, sitting through a working day or simply walking without pain. Your treatment plan is built around those goals and adjusted as you improve.
Recovery works best when treatment continues at home, so you will be taught exercises and habits you can follow between sessions. For many common problems such as stiffness, muscle strain or posture-related pain, you can start with a physiotherapy assessment directly. If your pain followed a significant injury, is severe, is getting worse, or comes with numbness, weakness, fever or weight loss, it is sensible to see a doctor first to rule out conditions that need medical treatment. After surgery or a fracture, always follow your surgeon's instructions on when and how to begin.
It depends on the condition, how long you have had it, your general health and how consistently you follow the exercise programme. A recent muscle strain may settle within a few sessions, while recovery after surgery, stroke or long-standing pain usually takes longer. Your physiotherapist will give you an estimate after the first assessment and review it as you progress.
Treatment should not be unbearably painful. You may feel some discomfort while stiff or sore areas are stretched or loaded, and mild muscle soreness for a day after new exercises is common. Tell your physiotherapist if anything feels too intense, as techniques and exercises can always be adjusted to a level you can tolerate.
For most back pain, staying gently active is better than prolonged bed rest, which tends to stiffen the back and slow recovery. Short periods of rest to settle severe pain are fine, followed by a gradual return to walking and light movement. Your physiotherapist can show you which movements are safe and helpful for your specific problem.
Often sooner than people expect. Gentle movement and exercises for the surrounding joints may begin very early, while loading the healing area is introduced in stages according to your surgeon's or orthopaedic doctor's instructions. Starting the right exercises at the right time helps prevent stiffness and muscle loss, so ask your doctor when you can begin.
Do not wait for an appointment if back pain comes with numbness around the groin or inner thighs, difficulty passing urine or stool, or rapidly worsening leg weakness; if you have sudden weakness or numbness on one side, a drooping face or slurred speech; chest pain or severe breathlessness; a swollen, painful, warm calf after surgery or long immobility; or a severe injury with deformity or inability to bear weight. Call the emergency number 112 or go to the nearest hospital emergency department straight away.
